Dr Ahmed Al-Obaidi leaving

Dr Al-Obaidi will be leaving the practice at the end of March. We wish Dr Al-Obaidi all all the very best for the future and thank him for all his hard work. We are currently trying to recruit a new Dr.

Patient access to their GP record

Through SystmOne Online patients can make a request to be able to view their detailed patient record in coded form.

NHS England - List Maintenance Project 2015

Registration letters from NHS England – please respond if you get one.

The NHS is carrying out work in Bath, Gloucestershire, Swindon and Wiltshire to make sure the patient details on GP lists are accurate. Letters are being sent to some patients, but not all.

If you do receive a letter, please either confirm your details or submit any changes.

Electronic Prescription Service

This service gives you the option to 'nominate' the place you choose to get your medication from so that they can receive your prescriptions electronically. This could save you a trip to the surgery just to collect a prescription form - particularly if you have regular prescriptions.

Online booking appointments and requesting repeat prescriptions

It is now possible to book and cancel appointments and request repeat prescriptions online. Please collect a form from Reception to register. You will be asked for proof of identity.

Out of Hours

On weekdays between 6.30pm and 8.00am, and on weekends and bank holidays, access to Out of Hours services is now via NHS111 - please dial 111. After assessment of your symptoms you may be invited to attend the Out of Hours Centre which is at The Great Western Hospital site and is operated by SEQOL.

Appointments

Although we run an appointments system, some appointments do overrun and delays do sometimes occur. We appreciate that this may cause inconvenience at times. Please be patient; you may require the extra time one day.

 Children's Urgent Clinic

The Children’s Clinic is for people aged from 3 months to 18 years old, who are unwell and can’t get an urgent GP appointment. The clinic is open from 8am to 8pm, Monday to Friday, please telephone for an appointment on the day. Call 01793 646466

Patients can telephone between 8am and 6.30pm to book an appointment. Appointments can be booked up to 6 weeks ahead.

When telephoning the surgery you will be offered the next available routine appointment. If you feel your problem is more urgent and need to be seen before the offered appointment then, where possible, you will be either be offered an appointment that day or a telephone appointment with a Dr.

Same day appointment requests are for those patients whose medical problem must be dealt with on the same day and cannot wait until the next routine appointment. When booking same day appointments the Doctors have asked the Receptionists to take a brief summaryof the problem so the Doctors can prioritise emergency medical problems.

You can help us to help you by:

  • Trying to consider if the problem you have had has been a longstanding one which is suitable to be booked ahead.
  • Ensuring that if you cannot attend an appointment, or no longer require it, that you cancel it as far in advance as possible.

Although we run an appointments system, some appointments do overrun and delays do sometimes occur. We appreciate that this may cause inconvenience at times. Please be patient; you may require the extra time one day.

Children

In addition to appointments at the surgery there is a Children's Urgent Clinic based at Moredon Medical Centre which is by appointment only Monday to Friday 8am to 8pm. Appointments can be booked by telephoning 646466. Appointments are held at:
