News on Dr’s

Dr Al-Obaidi has left the practice.

Dr Weir is on Maternity leave.

We are having difficulties recruiting new Dr’s.

Please bear with us during this difficult time.

Urgent care

You can no longer book an urgent appointment on the day at 8am.

Instead the Receptionist will take your details and the Dr will telephone you.

The Dr will then offer you an appointment if they think you need to be seen in person.

You may be offered an appointment at SUCCESS (Swindon Urgent Care Collaborative Emergencies Surgery Scheme).

SUCCESS is based at Moredon and Hermitage.

Children’s Clinics

Children aged 13 weeks to 18 years of age, can be seen at SEQOL’s Children’s Clinics, sited at the Moredon Medical Centre and at Carfax Health Enterprise.

Open from 8am to 8pm seven days a week by appointment.

Telephone 01793 646466 to book an appointment.

Children can also be seen at the Urgent Care Centre at GWH 24 hours a day.

Patient access to their GP record

Through SystmOne Online patients can make a request to be able to view their detailed patient record in coded form.

NHS England - List Maintenance Project 2015

Registration letters from NHS England – please respond if you get one.

The NHS is carrying out work in Bath, Gloucestershire, Swindon and Wiltshire to make sure the patient details on GP lists are accurate. Letters are being sent to some patients, but not all.

If you do receive a letter, please either confirm your details or submit any changes.

Electronic Prescription Service

This service gives you the option to 'nominate' the place you choose to get your medication from so that they can receive your prescriptions electronically. This could save you a trip to the surgery just to collect a prescription form - particularly if you have regular prescriptions.

Online booking appointments and requesting repeat prescriptions

It is now possible to book and cancel appointments and request repeat prescriptions online. Please collect a form from Reception to register. You will be asked for proof of identity.

Out of Hours

On weekdays between 6.30pm and 8.00am, and on weekends and bank holidays, access to Out of Hours services is now via NHS111 - please dial 111. After assessment of your symptoms you may be invited to attend the Out of Hours Centre which is at The Great Western Hospital site and is operated by SEQOL.

Appointments

Although we run an appointments system, some appointments do overrun and delays do sometimes occur. We appreciate that this may cause inconvenience at times. Please be patient; you may require the extra time one day.

Free NHS Health Checks

The Practice provide for al patients in the age group of 40 to 74 years of age a free NHS Health Check.

Everyone is at risk of developing heart disease, stroke, diabetes or kidney disease. Good news is that these conditions can often be prevented, even if you have a family history.

Our Health Checks are 30 minutes in length, where our HCA will ask a few simple questions, we will record your height, weight, age, sex and ethnicity. We will also record your Blood Pressure and check your cholesterol levels.  We will advise you of the outcome of your check, and provide you with advice on healthy lifestyles.

You can help the process by completing the relevant Health Check Questionnaire, which you should bring with you, after you have made your appointment.
